Sudbury – Funded by eHealth Ontario, the new North East CCAC-led e-Notification information-sharing project, which has been in full operation at 24 Northeastern Ontario Network (NEON) hospitals since March 31, won the System Partnership Award at the Awards for Excellence Banquet held in Toronto, Thursday, May 28th, 2015. The awards were part of the 2015 Conference of the Ontario Association of Community Care Access Centres (OACCAC).

The e-Notification software is a simple but valuable tool. When a patient presents to the Emergency Department (or is admitted to an inpatient unit), their name and birthdate are entered in the hospital’s computer system. This information is relayed to a provincial database, and if there is a match, both the hospital and CCAC systems are notified.

 “This project has seen an unprecedented level of cooperation among partners for the benefit of our patients,” says Peter Taylor, e-Notification Steering Committee Chair, and North East CCAC Director, Business Solutions. “e-Notification enables patients to experience smoother transitions, faster discharges, fewer readmissions to hospital, and better follow-up care.” 

As System Partnership winners, selected team members will also be invited to present at the Canadian Home Care Association’s (CHCA) International Home Care Summit in Ottawa in November, profiling the e-Notification project to delegates from across Canada and around the globe, and being featured in a special CHCA report to be shared Canada-wide.
