The Province has established specific voter ID requirements for the election. An original or certified or notarial copy, of a document showing the person’s name, qualifying address and signature is required. A list of acceptable forms of ID is as follows:
An Ontario driver’s Licence.
An Ontario Health Card (photo card).
An Ontario motor vehicle permit (plate portion).
A mortgage, lease or rental agreement.
An insurance policy.
A document issued or certified by a Court in Ontario
A loan or financial agreement with a financial Institution
Any other document from the government of Canada, Ontario or a municipality in Ontario or from an agency of such a government.
Any document from a Band of Council in Ontario established under the Indian Act (Canada).
