The Ontario Provincial Police (OPP) has completed its investigation into the fatal motor vehicle collision involving a tanker truck that spilled formaldehyde on Hwy 63 on May 21, 2012.

The OPP Technical Traffic Collision Investigation Unit (TTCI) investigation has determined that the collision was caused by the driver not being able to negotiate the curve.

OPP TTCI investigators were unable to perform normal safety inspections on the vehicle due to contamination issues and as a result any mechanical issues cannot be ruled out.

The investigation has revealed that the tractor trailer unit did make attempts to right the unit from entering the ditch, but was unable to correct its course, consequently causing the collision. It cannot be confirmed as to what caused the driver to be unable to negotiate the curve.

The driver of the tractor trailer died as a result of injuries sustained in the collision itself, and not as a result of formaldehyde exposure.

The OPP investigation is complete.
